Forms

The links to the following forms are provided for your convenience. Information on these pages will always be superseded by the paper forms available from the SCA Corporate Offices in Milpitas, CA. Those versions of documents are authoritative for all purposes.


Minor forms

Minors are defined in Florida as persons under 18 years of age. We do NOT have notaries on site, ever! Forms must be notarized in advance.

Minor Attendance

Minors must be accompanied by a parent/guardian or their authorized surrogate at all times. Minors must have on-hand a completed, signed, and notarized minor waiver form to attend an SCA event. The minor waiver form is on the Society Forms page; do a search for "minor waiver".

Minor Combat

Minors are allowed to participate in certain forms of combat, when a completed, signed, and notarized minor combat form is present. The minor combat form is on the Society Forms page; do a search for "minors' combat waiver".

Non-Member Adult forms

Join the SCA

You can join the SCA using the handy online membership form. You can also download paper membership forms and send it in through the postal service.

Non-Member Attendance

If you are not a member of the SCA and wish to get into an SCA event, you will be required to sign an adult waiver. This form does not need to be notarized or signed in advance, but if you wish to see the form it can be found on the Society Forms page; do a search for "adult waiver".

Horses

Oldenfeld occasionally hosts equestrian events; limited stable space is available. If you bring a horse - or intend to ride on someone else's horse - you must be willing and able to sign the equestrian waiver. This form does not need to be notarized or signed in advance, but if you wish to see the form it can be found on the Society Forms page; do a search for "Adult Equestrian Waivers".

Arts & Sciences Forms

If you wish your research or re-creation to be evaluated in an ArtSci competition, you should be familiar with the "Art/Sci Standard Entry Packet".

Youth are encouraged to submit work for ArtSci, via the age-specific "Youth Art/Sci" form.
